I have created some video, so I want to know that how to include music Copyright Disclaimer in our Youtube Video. Please assist me.

You will need to get permission/licence from the rights holder. That is all you need (and since they know that they have granted you licence- you obviously do not need to advertise that they granted you licence).

Again- "copyright disclaimers" are pointless.

That is because they are no substitute for licence/permission and if you were to get caught (and most rights holders would know to check YouTube first), you would actually be letting them know that you know you violated their copyright but don't really care enough to get permission and think that putting up a notice to let everyone know what you did is alright. Which might actually be worse than making a mistake and "accidentally" violating their copyright.

Folks-

Using someone else's content without their permission is copyright infringement.

And copyright infringement is a legal matter, where the copyright owner has the right to ask YouTube to not let you infringe their copyright.

YouTube has to follow the law and remove your video with their content or put adverts on it so that the copyright owner gets paid for you having their content in your video.

Putting a disclaimer on a video is not good enough. It's a bit like a thief putting a note in the window of the car they are driving around in saying that "it's not my car, but no theft intended".

Filing a dispute/counter-notifying a copyright claim when you do not own the copyright or do not have permission from the person(s) who does/do- that is a legal matter too. It is called "perjury" (legalese for lying), and YouTube need to take a rather dim view of it. In other words- you perjure yourself on the legal form? YouTube will find out and they will suspend you for misuse of their (legal) form.
